This is a review of past medical conventions. The 8th medical convention was organized in the year 2004. In less than six years there have been several reforms: the reform of health, with diverse laws regulating upon the health system (including creation of the SNIS, FONASA, separation of ASSE’s management of the MSP). A law for the creation of the national medical school of the Uruguay is has been finally issued. These matters had been considered and discussed between the 1999 and 2004 conventions.
We are now confronted with a different picture. The health system reform laws have been approved. The law on the Medical College also has passed, although with difficulty.
Since the generations of medical leaders have varied substantially, it is possible that those who today occupy seats of leadership have not participated in previous instances, at least for the most part, and that others already do not remember some of the many aspects to this rich heritage of national medical thinking has generated, and partially gone into oblivion. The so mentioned change in the model of attention is still waiting to get the train that brings it. Meanwhile we continue training doctors for the previous model, there are specialists who shine by their absence (or because migrated best horizons) and the misunderstandings, even with the reform, are produced. The inherited Centralism follows so rampant, back to the reality of the country, which for the most part, State leaders know little about.
Is it not here who gets the applause and acknowledgment by being devoted to something that was in the spirit of the national doctors over many decades. In the same way that many collective doctors in other countries around the world have worked to make increasingly more comprehensive, universal, equitable and human health care. That has become one of the most significant contributions of the medical profession around the world. On the contrary, it would seem that it is sought in some sectors next these historic contributions, to attribute merits that are foreign to them. It is the case of the rook dressed in feathers of the Peacock.
Our time, from the last decades of the 20th century, after the dictatorship, was marked in the field of health by facts that generated concern, between 1986 and 2003.
The old structures of a health care system broke down and were brought to ruins, such as public hospitals, or Mutualism with the consequent loss of jobs; the chances of employability in a more widened and less supportive labor market deteriorated. The technological and scientific advances threaten to destroy the doctor, attitude and fitness clinics and doctor-patient relationship, at the same time that moved it from the central place that the professional had on the generation and management of services and proposals; the training of professionals in excess (or deficit) in relation to the demands of the structure of health services; the defense of medical ethics, as well as professional journalism return to foreground when colleagues, relations with institutions or patients suffer tremendous impacts and head looks toward collective mechanisms of moral safety (ethics) and material (conditions of work and remuneration).
